There have recently been questions about IEPs for those starting Pre-K (including how and/or should you include toileting goals). I'm sure there may be flaws in Janna's but I think it was a pretty good one (written by her teachers/therapists with our input). Annual Goal

Janna will increase independence with toileting skills to improve overall age appropriate self-help skills.

Benchmarks

For 5 consecutive sessions, Janna will:

1)Push down pants with prompting during toileting times at home and school.
2)Pull up pants after toileting with adult prompts and cues.
3)Sit on potty chair a minimum of two minutes to facilitate frequent voids in potty.
4)Remain dry between toileting intervals at school.
5)Wash hands, dry hands and dispose of paper towel as indicated on the hand washing chart with teacher’s assistance.
6)Will increase successful voids in potty by 50 percent.
7)Will follow a timed-toileting schedule at school and home with no more than one accident per day.

How progress will be measured:

Teacher observation, potty charts; parental report from home.

Annual Goal

Janna needs to increase her self-help skills in order to increase independence with dressing skills.

Benchmarks

For 5 sessions, Janna will:

1)Zip hooked zippers in training vests without assistance.
2)Unbutton and button large button on training boards with full support to complete the task.
3)Put on training vest or similar item with teacher support.
4)Put on coat with teacher’s assistance.
5)Zip her coat or jacket after it is hooked.
6)Pull on socks after placed on feet with prompts and assistance.
7)Push on shoes after being placed on her feet with assistance and prompts.

How progress will be measured:

Teacher observation, data sheets; and parent input.


Annual Goal

Janna will improve self-feeding skills to increase overall independence with self-help abilities.

Benchmarks

For 5 consecutive sessions, Janna will:

1)Feed self with spoon by participating in hand-over-hand assistance during snack at school and meal times at home.
2)Carry loaded spoons to her mouth with adult prompting during snack and meal times.
3)Scoop food with spoon with adult prompting during snack and meal times.
4)Drink from a regular cup without a lid with full assistance.
5)Drink from a regular cup without a lid with adult prompting and minimum assistance.

How progress will be measured:

Teacher observation and notes during school snack times.
Parental report from home.

Annual Goal

Janna will increase socialization, play, and attending abilities to develop overall age appropriate social skills.

Benchmarks

For 5 consecutive sessions, Janna will:

1)Imitate simple motor actions and body movements during songs and fingerplays with minimal assistance.
2)Participate in turn taking activities during group time with teacher support and prompting.
3)Make activity choice when given two items with prompts and using sign language and verbalizations.
4)Identify her printed first name with visual cues.
5)Transition between familiar classroom and school activities using transitional cues/supports.
6)Demonstrate appropriate use/play with objects and toys and will decrease mouthing/chewing of inappropriate items with teacher supervision.
7)Say/sign first name with teacher prompting.


How progress will be measured:

Teacher/staff observations, notes and records; and parental report from home.

Annual Goal

Janna will increase matching, color identification and object/picture receptive and expressive abilities to improve overall age appropriate knowledge skills.

Benchmarks

For 5 consecutive sessions, Janna will:

1)Match identical objects with teacher support.
2)Match objects to pictures with support.
3)Match like pictures with teacher’s prompts and cues.
4)Match 4 colors with minimum support.
5)Will give requested color from choice of 2 with teacher support.
6)Will match circle, square, and triangle shapes with teacher support.
7)Give requested objects with cues.
8)Give requested picture with cues.

How progress will be measured:

Teacher observation, data sheets; parental input.

Annual Goal

Janna will increase manipulative, dexterity and strength to improve overall age appropriate fine and visual motor skills.

Benchmarks

For 5 consecutive sessions, Janna will:

1)Put together 10 sets of pop beads with prompts.
2)Stack 3 to 5 blocks with adult prompting.
3)Will complete 3 to 4 shape form boxes with assistance.
4)Will place 5 pegs in resistive pegboard with assistance.
5)Will complete start to finish activities with minimal assistance after initial prompting.
6)Make circular scribble with crayons or markers with assistance.
7)Imitate vertical and horizontal lines with prompts.
8)String jumbo beads with adult cues.
9)Nest graduated cups up to 5 sizes.


How progress will be measured:

Data, anecdotal records, teacher observation

Annual Goal

Janna will ambulate independently over all surfaces and elevations.

Needs: dynamic balance reactions, increased proximal stability, independent ambulation over all surfaces and elevations.

Benchmarks

1)Janna will stand from the floor using a half kneel posture x3:3 in 3 consecutive sessions.
2)Janna will step on and off a 2-4 inch step independently x3:3 in 3 consecutive sessions.
3)Janna will ascend steps using one handrail placing both feet on each step independently in 3 consecutive sessions.
4)Janna will descend steps using one handrail placing both feet on each step independently in 3 consecutive sessions.
5)Janna will ambulate independently over unlevel surfaces, stepping over obstacles in her path in 3 consecutive sessions.
6)Janna will jump up clearing both feet off the floor 3 consecutive sessions.
7)Janna will jump down from a 2-4 inch step landing on two feet with contact guard assist in 3 consecutive sessions.
8)Janna will propel herself forward on a scooter board 20 ft. with prompting in 3 consecutive sessions.
9)Janna will throw a ball overhead with both hands with direction 2-3 ft. in 3 consecutive sessions.
10)Janna will kick a rolling ball 3 ft. with physical prompting in 3 consecutive sessions.

How progress will be measured:

Daily treatment data.

Annual Goal

Janna will meet success with identified objects for improved functional communication skills.

Benchmarks

Janna will in 4 of 5 trials over 3 sessions:
1)Identify at least 25 nouns by name via pointing.
2)Increase understanding of at least 10 action words via pointing to presented stimuli.
3)Identify at least 5 articles of clothing on self or presented model.
4)Follow verbal requests embedded with positional words (eg. in, out, on , off, under, top)
5)Identify at least 20 nouns by function by pointing.
6)Label presented pictures/objects via verbal approximation and/or manual sign
7)Produce noun+verb/verb+noun phrases (eg. Eat cookie, girl walks etc.) via verbal approximation and/or manual sign

How progress will be measured:

Data obtained from therapy notes, observations, parent/teacher input.

Annual Goal


Janna will increase upper extremity and hand strength; increase fine-motor skills to improve developmental skills for school performance.

Needs: UE and hand strengthening, stacking blocks, puzzles, bilateral skills, copying strokes, and utilizing a consistent digital grasp.

Benchmarks

1)Janna will complete tasks such as propelling scooter board and wheel barrel walking to increase upper extremity strength.
2)Janna will perform mega-blocks, snap beads, and thera-putty activities to increase hand strength.
3)Janna will stack a tower of 4-5 (1”) blocks with prompts.
4)Janna will complete a 4-5 piece inset puzzle independently.
5)Janna will copy a vertical and horizontal stroke after demo with prompts.
6)Janna will utilize a 2-3 digit grasp for 75% of fine-motor activites.

How progress will be measured:

Direct OT intervention, documentation, and observations.

Thank you for posting! Wow that is a lot, and I have been pushing for more goals. Luke doesn't have that many, but they started out with about 6 goals for the year. Now he has more than 20. I am going to print this out and keep it for Luke's fall IEP meeting. Some are already in his, but there are more I would like included. Aim high!

Thank you so much for this post. We had Jacey's IFSP today, and began talking about transitioning (starting in May). One of the things I want included is potty training and our service coordinator told me that would not be included in an IEP. I'm glad to see that it can be.... Thank you, thank you, thank you....

Nice! Those are some great goals!!! It's funny, I was just talking to a mom who is in a different state and going over her son's IEP. Your IEP is kinda like hers which throws me for a loop. Your goals are "general" and your benchmarks are specific and different goals within the general goal...but for our stuff, some of your benchmarks ARE our single goals. So a goal would be say "zipped hooked zippers" for Russell's and the benchmarks are how that one goal would be measured.

I am trying to get use to looking at other states/districts IEP formats. The format we have is how it's been no matter where we have been.

Nevertheless, I like the detail with your benchmarks, that is key no matter what! The less detail and more generic, the less they can be held accountable!

Thanks for sharing the details of your daughter's IEP. Those reports overwhelm me with so many repetitive questions.

This will help tremendously to clearly explain what are my expectations for my Xavier. Should be starting pre-pre-K in the fall....he turns 3 end of June, seems like perfect timing for school.
